The Community Health Innovation Region (CHIR) model is being implemented in five communities throughout Michigan. These include:

• Genesee County 
• Jackson County
• Livingston and Washtenaw Counties
• Muskegon County
• Northern Michigan (including Antrim, Benzie, Charlevoix, Emmet, Grand Traverse, Kalkaska, Leelanau, Manistee, Missaukee, and Wexford counties)
