About the CHIR

Northern Michigan received designation as a Community Health Innovation Region (CHIR) as part of the State Innovation Model (SIM). This designation enables federal support for innovative developments to develop community health assessment planning and data analysis to assure progress in project focus areas for the Medicaid population:

  • The region will have a unified health improvement plan.
  • Clinical and community linkages will be used to impact health outcomes.
  • Collective impact and community health impact models will be used to improve health outcomes.

The Northern Michigan Public Health Alliance is acting as the backbone organization for this project. Northern Michigan Health Coalition and several of its members actively participate in this development.
